Aubrey Plaza and Jeff Baena were separated for four months before the latter’s tragic death.

According to a medical examiner’s report, Baena, 47, made “concerning remarks” to Plaza, 40, that “prompted her to call a friend to perform a welfare check on her husband.”

“Jeffrey Baena was separated from his wife [redacted] since September 2024 when she moved to New York,” the report, released on Wednesday, states. “The descendant had a phone conversation with his wife last night.”

Authorities spoke to Plaza on the phone on Jan. 3, and she “confirmed” the separation, the report adds.

Lieutenant Kelsey Weber revealed, “Baena had been attending therapy since the event.”

The officer also noted that the screenwriter did not have any “previous suicide attempts” but had “difficulty sleeping.”

“[Redacted] last knew her husband to be alive when he sent her a text message on 01/03/2025 at 1036 hours (unverified Eastern Standard Time),” the report concludes.

As OK! previously reported, the Horse Girl filmmaker’s assistant discovered his unconscious body on January 3 at his Los Angeles, Calif., home.

Although Baena and Plaza kept their romance extremely private, they collaborated on many projects such as Life After Beth, The Little Hours and Spin Me Round. “She’s awesome. I’d be working with her if she wasn’t my wife, but luckily, she is my wife,” the late director gushed in a 2022 interview. “The opportunity to do something creative [together] where we’re both fulfilled — how rare is that?”

In December 2021, the Parks and Recreation star revealed during an appearance on The Ellen DeGeneres Show why she and Baena finally decided to make things official after a decade of dating.

“We got a little bored one night,” she explained. “We got married and I’ll tell you how: Onehourmarriage.com. That’s real, look it up.”

Plaza said in January that Baena’s death was an “unimaginable tragedy.”

“We are deeply grateful to everyone who has offered support,” The White Lotus star said in a statement along with the late writer/director’s family. “Please respect our privacy during this time.”
